Create a Survey Form with MCP, Then Have AI Read the Answers

After a class, most students will happily tell you what worked and what did not — if asking takes them a minute and reading the answers does not take you an evening. Google Forms can do the survey part. What this guide adds is doing the whole thing from the AI assistant you already use: describe the survey in chat, publish it, and later ask the same assistant what people said.

The example is a small neighborhood yoga studio collecting class feedback. FormEdge is connected to ChatGPT or Claude over MCP, so the AI can build the form, publish it, and read the responses.

What you end up with

A single survey form at a link you can share:

• A 1–5 rating for today's class, and a 0–10 “would you recommend us” question
• Which class they took, and what they liked — both with an “Other” option they can type into
• Two open questions: a class time they wish you offered, and anything else on their mind

No name or email fields, so students can be honest. You share the link by email or text after class, or on a QR code at the front desk.

What the AI produces is a draft. The questions and choices come from what you tell it, and it may add a question or two you never asked for, so read the list before it goes live.

Before you start

• A ChatGPT or Claude account
• A FormEdge account (free to create from the end of this page)
• A short list of what you want to ask — aim for five to seven questions

Decide how each question should be answered before you ask for the form:

A feedback survey people finish on their phone in the parking lot beats a thorough one they abandon. Mostly multiple-choice questions also make the results easier to add up.

Walkthrough

1. Create an account and connect your AI

   Sign up for FormEdge, then connect it from the assistant you use and approve what the AI may do. The connection steps, including the approval page, are covered in the website guides:

   Grant create and edit so the AI can build the form. Leave publishing out if you would rather publish yourself; you can add it later. Reading submissions is already on by default, which is what lets the AI read the responses later.

2. Describe the survey

   Swap in your own studio, classes and wording.

   Questions, in this order:
   1. How was today's class? (rating scale 1–5, "Poor" to "Excellent", required)
   2. Which class did you take? (pick one: Morning Flow, Lunchtime Express, Gentle Yoga, Evening Yin, plus an "Other" option they can type into; required)
   3. What did you like? (check all that apply: the instructor's cues, the pace of the class, the music, the studio space, the small class size, plus "Other")
   4. How likely are you to recommend us to a friend? (scale 0–10, "Not at all likely" to "Extremely likely", required)
   5. Is there a class time you wish we offered? (short text)
   6. Anything else you'd like to tell us? (long text)
   
   Keep it anonymous: no name or email fields.
   After submitting, show "Thanks for the feedback!" with "We read every response and use it to plan our classes. See you on the mat soon."
   
   Don't publish it yet. Show me the questions as a list first.

3. Check the questions

   Read the list the AI sends back the way a student would:

   • Question wording and choices exactly as you wrote them
   • Nothing extra — a name or email field, a question you did not ask for
   • The right questions marked required
   • Scale labels on the right ends (1 is “Poor”, 5 is “Excellent”)

   Send corrections into the same conversation:

   Add "Weekend Slow Flow" to the class list.
   Make the recommend question optional.
   Add a placeholder to the class-time question: "e.g. 6:30 AM on weekdays".

4. Set a closing date or a response limit (optional)

   If the survey is tied to a season or a workshop series, set it to close on its own. Once the date passes or the limit is reached, the form stops accepting responses.

   Stop accepting responses after (e.g. 150) responses.
   When it's closed, show: "This survey has closed. Thanks to everyone who shared feedback!"

   For an always-open feedback form, skip this step.

5. Publish and share the link

   Open the link on your phone and answer it once yourself before sending it out. Then share it where your students already are:

   • In your class reminder emails or texts
   • In your Instagram story or link in bio
   • On a QR code at the front desk (the form's editor in the FormEdge dashboard shows a QR code for the public link)

6. See exact numbers in the Summary view

   Responses are listed under Submissions in the FormEdge dashboard. Switch the view from List to Summary to see each question added up:

   • Pick-one and check-all questions: count and percentage for each choice, with examples of what people typed under “Other”
   • Rating scales: how many picked each score, and the average
   • Open text: the latest answers

   When you need numbers you can quote — in a newsletter, or to decide whether to keep a class — use this view.

7. Have the AI read the responses and pull out themes

   Back in the chat, the AI can read the responses for you. This is most useful for the open-text answers, where the dashboard shows only the latest ones.

   Export the responses to the class feedback survey as CSV and summarize them.
   Give me the average rating, what people liked most, and the themes in the open comments — especially anything we could fix.

Common snags

When you're done collecting

To stop a survey that has no closing date, ask in chat — “stop accepting responses to the class feedback survey” — or unpublish it from the dashboard (asking the AI to unpublish needs the publish permission). The form and every response stay in your account.
